Here you have to choose Paper Based Test if you are applying for the first time.

Now you have to choose Under 5 Year if you are New applier. Similarly, if you want to extend your 5 years visa, re-apply for Work Visa in Korea. Then choose over 5 years.
After this you have to choose Approve. Then you will be redirected to a new web navigation.

So, you will get the option to pay for the form via ImePay, E-Sewa, FonePay, Global Smart, Global Online. Or you have the option to pay the bills at the Global IME Counter and for this simply click on Next.
After you pay bills you will get the payment code for online payment or voucher number for counter payment.

Here, you have to enter the Voucher Number which you received from Global IME counter after making payment of 24 USD.
Then you have to enter your Password Number which should be exactly correct.
After entering the Voucher Number and Password Number you have to click on Next.
So, when you click on next then you will be redirected to the form fill up of EPS Korea.
